There are cases when the decision of an expert team assessing a person's daily functioning contains inaccuracies. Sometimes the cost of such a mistake can be very high, because the extract from the expert team's decision is an official document on the basis of which payments, rehabilitation, social support, etc. are issued.
The Ministry of Health of Ukraine on its Facebook page on Telegram explains what to do in this case.
What can be corrected (typos):
- incorrect spelling of the full name;
- errors in the date of birth or other important dates;
- incorrect gender;
- errors in the address or e-mail;
- inaccuracies in the series/number or details of documents;
- other material details that affect the content.
They do not correct: style, punctuation, cases, language variants that do not change the meaning.
Who can apply:
- the person in respect of whom the decision was made;
- an authorized representative (a document of authority must be attached).
How to apply:
- a short statement in any form - indicate what exactly needs to be corrected;
- time limit - 40 calendar days from the date of receipt of the extract;
- submit to a healthcare facility where an expert team works.
The institution reviews the application within 7 business days and either instructs the team to make changes or notifies you of the decision. After the correction, you will receive an updated extract (and, if necessary, updated recommendations for the IPR).
If an error is discovered by the expert team itself, it can correct it within 10 calendar days after the decision is made and send an updated extract to you and the institutions that received it.
Tip: Immediately after receiving the extract, check the name, date of birth, address, document numbers, and other key data.
