If the pension certificate is lost, the pensioner can obtain a new document by contacting the selected service center of the territorial body of the Pension Fund of Ukraine or online - through a personal account on the Pension Fund of Ukraine's electronic services web portal (portal.pfu.gov.ua) or using the Pension Fund mobile application.
- When applying to the service center, a pensioner must present a passport, a certificate of identification number and fill out an application for a pension certificate, to which he or she must attach a color photo of 3×2.5 cm.
- To apply for a pension certificate through the Pension Fund's electronic services web portal, you need to go to the section “On pension provision” and select the option"Application for the production of a paper pension certificate".
- If you submit an application from the Pension Fund mobile application, select the tab “To the FIU”:section “Communications to the Pension Fund of Ukraine” - “Application for the production of a pension certificate”.
To apply for a certificate through the Fund's electronic services, you must prepare scanned copies of your passport, certificate of identification number, signature (in black ink) and a photograph. Scanned copies must be color and clear; saved in JPG or PDF formats. The size of each file should not exceed 1 MB, and the names should correspond to the type of scanned document.
The pension certificate (after production) will be issued to the person at the service center of the Fund, which is indicated in the application.

The electronic display of a pension certificate in the Diia mobile application generated after the pension certificate is issued has the same legal force as the traditional one.
