Declaration with a doctor - online

February 20, 2026

You no longer need to go to the hospital to submit a declaration to your doctor or update your phone number in the electronic healthcare system. The Ministry of Health of Ukraine on its website announces that the beta testing of the patient's office has been successfully completed - the digital platform has been tested with patients and doctors and is ready for expansion.

The account is available for all adult Ukrainians, as well as for teenagers aged 14 to 18 with their own electronic signature. In particular, adolescents can log in to the account and view their data independently, and, subject to confirmed civil capacity (and availability of relevant documents), submit a declaration and make the necessary changes to their personal data as needed.

What is already available in the Patient's Personal Account?

  • review and update personal data in the electronic healthcare system;
  • reviewing current and previous declarations with a doctor;
  • search for a doctor and submit a declaration online - without visiting a medical facility;
  • managing access to electronic accounts that you have authorized to process your personal data.

Important: Your personal data and declarations are now available in the patient's account. Access to the medical record (tests, prescriptions, and diagnoses) will be available gradually in the next stages.

How to start using it?

The patient's office is accessed through special eCabinets - choose the one you prefer to start. This can be an eCabinet developed by the state (eCabinet of the National EHR System) or by a business (patient information system).

You can choose any account - their main features are the same, but private accounts can offer their own additional functions. Over time, there will be even more of them! You can view the list of all verified e-cabinets and choose the most convenient one by via link.
